Terms and Conditions including Parental Waiver and Consent for Free Trial, Programs or Camps etc.
With the confirmation of your child’s registration for our free trial, or programs, or camps, or any activities organized by Ministry of Football Pte Ltd (hereinafter “MOF”) and / or its related entities such as MOF Academy (hereafter “MOF activities”) and /or (partial/complete) payment of the requisite fee, please note that you hereby agree that you give full consent and approval for your child to participate in any MOF activities.
You understand that there are certain risks of injury inherent in the practice and play of football as well as other related activities incidental to your child’s participation. You are willing to assume these risks on behalf of your child and you hereby certify that your child is fully capable of participating in any MOF activities and is healthy and has no disabilities or infirmities that would restrict full participation in these activities, except as listed above.
In addition to giving your full consent, you do hereby waive, release and hold harmless the organisation, its officers, coaches and representatives for any injury that may be suffered by your child in the normal course of participation in any MOF activities and the activities incidental thereto, whether the result of negligence or any other cause.
You agree further that any/all photography of your child shall be property of MOF, to be utilised by MOF as it deems fit, for objectives consistent with its commercial goals. Finally, please note that insurance is not provided for your child. If you wish to insure your child for outdoor sporting activities kindly contact any reliable insurance companies (or if you require a recommendation from us please let us know).
You note as well that the participation fees for MOF activities are non-refundable and that any cancellation to your child’s participation will result in a forfeiture of the full payment made prior. Sessions missed can be utilised at another date so long as consumed within 4 weeks of the missed sessions. You acknowledge as well that there may be amendments made to these MOF activities, comprising free trials, programmes and/or camps (such as venue and timing) and that you will be informed by the organisation of any of these said changes.
Should any parent, adult and/or child be discovered participating in any MOF activities without having first made full pre-payment, the parent or adult shall make full payment of a regular course amount within 24 hours of such discovery. MOF also reserves the right to impose a late/overdue/unsanctioned fee of $45 per such session attended.
The participants as well as any adults (which include guardians, parents etc) accompanying the children during training or competition sessions must agree to comply with good behaviour: no demanding of the coach or venue IC to amend their instruction or coaching behavior whilst the lessons is going on, and under no circumstances should adults (parents) raise their voices at or appear threatening in any way to any MOF staff, coaches or assistants. Should any adult be deemed by MOF to be guilty of such actions, (a) MOF reserves the right to expel the adult & child from MOF activities without any refund, (b) the adult(s) shall indemnify MOF and reimburse MOF and be responsible for all medical costs and damages relating to such actions, (c) both the adult(s) and the child shall not be present at any MOF activities permanently from that point onwards – there will be a permanent ban on them, and (d) the person(s) shall compensate MOF damages quantified at a minimum of $500 per incident (and MOF reserves all rights to claim further damages as the circumstances warrant).
